The most common cause of this type of deformity is a late response to a minimal fracture of the proximal tibia. Children in the 12 to 24 month age though are not able to provide any history and if an unwitnessed fall occurred the trauma was likely missed. Usually though this causes enough symptoms such that most physicians would obtain an x-ray and see the fracture.
