If you strongly suspect that this is septic arthritis or osteomylitis than inpatient treatment with further work up and IV antibiotics is indicated. If the area the child lives in an area endemic for Lymes disease, this would be consistent and dischaging the child pending the outcome of the Lyme titer level would be acceptable. The antibiotic for Lyme should be oral doxycycline.
